If I equip a boltgun to replace the bolt pistol that comes standard on a commissar, do they lose an attack? I don't know if the attack is already added in the profile or do they gain an attack by having the CCW and Pistol and by changing to a boltgun they keep their statline.

No, the attack isn't already added you just don't add an attack.

To expand on this - the base profile is always represented in the stats.
So to take a couple of xenos race examples ; eldar and orks both get the option of having basic infantry either with a two-handed gun or a pistol and close combat weapon.
The basic ork boy with a shoota gets 2 attacks base, but if you decided to exchange the shoota for a slugga and choppa then they gain +1 attack. So a charging Ork boy gets 4 attacks; 2 base + 1 for the choppa +1 for charging Similarly Eldar guardians get the storm guardian option; the profile is the same but the storm guardians have shuriken pistol and close combat weapon, so they get the +1 attack.

Thanks guys for the replies, I think it was Straken (sp?) in fourth edition that had that written that his attacks were already included in his profile. I could be wrong and that always threw me off because I started in fourth edition.

I think Straken has a plasma pistol and shotgun anyway, so he doesn't qualify for the bonus, one way or the other.
Shotguns are Assault weapons but they need 2 hands to use, so it doesn't make any difference whether he uses the shotgun or pistol in cc.
The older model of Straken had his weapons in his hands, the newer one has a machete in one hand, so it gets confusing. But the machete means nothing in this instance.

I didn't play fourth but they did use to do that in third edition, for example with assault marine profiles. (I say without checking first).
