I think it would be really simple, just get a leman russ, and a basilisk, and switch cannons on them. Take the muzzle off the battle cannon, but it on the earthshaker, and glue it where the battle cannon should be.
Then with the basilisk, you build it as it should be, but instead of earthshaker you take the battlecannon without muzzle and glue it where the earthshaker should be.
And look! You'e got yourself a destroyer AND a Medusa mortar tank
